[ 摘要 ] 目的 探讨甲泼尼龙琥珀酸钠治疗儿童重症支原体肺炎的临床效果及对炎症因子的影响。 方法 选取 2022 年 4 月至 2023 年 3 月徐州矿务集团总医院收治的 100 例重症支原体肺炎患儿,依照随机抽签法分为两 组,每组 50 例。两组均接受常规对症治疗,对照组采用阿奇霉素治疗,实验组在对照组的基础上加用甲泼尼 龙琥珀酸钠治疗,比较两组临床症状消失时间、炎症因子水平、治疗总有效率。 结果 实验组患儿退热时间、咳 嗽消失时间、肺部啰音消失时间短于对照组(P < 0.05)。实验组患儿治疗后的白细胞、中性粒细胞水平低于对 照组,差异有统计学意义(P < 0.05)。两组治疗后的淋巴细胞、血小板水平比较,差异无统计学意义(P > 0.05)。 两组治疗后的 C 反应蛋白(CRP)、白细胞介素(IL)-4、IL-6、肿瘤坏死因子-α(TNF-α)、γ干扰素(IFN-γ) 水平低于治疗前,IL-10 水平高于治疗前(P < 0.05);实验组治疗后的 CRP、IL-4、IL-6、TNF-α、IFN-γ水 平低于对照组,IL-10 高于对照组(P < 0.05)。实验组治疗总有效率高于对照组(P < 0.05)。 结论 甲泼尼龙 琥珀酸钠治疗儿童重症支原体肺炎的临床效果显著,能够缓解临床症状,降低炎症因子水平,促进患儿康复。

[Abstract] Objective To study the clinical effect of Methylprednisolone Sodium Succinate in the treatment of severe Mycoplasma pneumonia in children and its influence on inflammatory factor. Methods A total of 100 children with severe Mycoplasma pneumonia in the General Hospital of Xuzhou Coal Mining Group from April 2022 to March 2023 were selected and divided into two groups according to random drawing method, with 50 cases in each group. Both groups received conventional symptomatic treatment, the control group was treated with Azithromycin, and the experimental group was treated with Methylprednisolone Sodium Succinate on the basis of the control group. The disappearance time of clinical symptoms, level of inflammatory factors and the clinical treatment efficiency were compared between the two groups. Results The time of defervescence, the disappearance time of cough and lung rales in the experimental group were earlier than those in the control group (P < 0.05). The levels of white blood cell and neutrophil in the experimental group were lower than those in the control group, the differences were statistically significant (P < 0.05). There were no statistically significant differences in the levels of lymphocytes and platelet between the two groups (P > 0.05). After treatment, the levels of C-reactive protein (CRP), interleukin (IL)-4, IL-6, tumor necrosis factor-α (TNF-α) and interferon-γ (IFN-γ) were lower than those before treatment, and the level of IL-10 was higher than that before treatment (P < 0.05). The levels of CRP, IL-4, IL-6, TNF-α and IFN-γ in the experimental group were lower than those in the control group, and IL-10 was higher than that in the control group (P < 0.05). The total effective rate of experimental group was higher than that of control group (P < 0.05). Conclusion Methylprednisolone Sodium Succinate treating severe Mycoplasma pneumonia in children has significant clinical effect, which can relieve clinical symptoms and reduce the level of inflammatory factors, and promote rehabilitation of children.
