[摘要]目的:分析妊娠期糖尿病(GDM)合并肥胖者患者血糖水平、肥胖指标与妊娠结局的相关性。方法:选取2021年1月至6月日照市妇幼保健院收治的100例GDM合并肥胖患者(观察组)及同期于日照市妇幼保健院查体的100例健康孕妇(对照组)作为研究对象。比较两组空腹血糖、餐后2h血糖、糖化血红蛋白、肥胖指标、新生儿与产妇妊娠结局,分析孕妇血糖水平、肥胖指标与新生儿及产妇妊娠结局的相关性。结果:观察组空腹血糖、餐后2h血糖、糖化血红蛋白、体重指数(BMI)、腰围(WC)、臀围(HC)、内脏脂肪指数(VAI)均高于对照组(P<0.05)。观察组产后出血、胎膜早破、早产、胎儿窘迫的发生率均高于对照组(P<0.05)。孕妇空腹血糖、餐后2h血糖、糖化血红蛋白、BMI、WC、HC、VAI与新生儿及产妇妊娠结局产后出血、胎膜早破、早产、胎儿窘迫呈正相关(P<0.05)。结论GDM合并肥胖患者的血糖水平、肥胖程度与妊娠不良结局呈正相关。

[Abstract] Objective To analyze the correlation between blood glucose level, obesity index and pregnancy outcomes in patients with gestational diabetes mellitus (GDM) complicating obesity. Methods A total of 100 patients with GDM complicating obesity admitted to Maternal and Child Health Care Hospital of Rizhao from January to June 2021 (observation group) and 100 healthy pregnant women who underwent physical examination in Maternal and Child Health Care Hospital of Rizhao during the same period (control group) were selected as the research objects. Fasting blood glucose, 2 h postprandial blood glucose, glycosylated hemoglobin, obesity indicators and neonatal and maternal pregnancy outcomes were compared between the two groups, and the correlation between maternal blood glucose levels, obesity indicators and neonatal and maternal pregnancy outcomes was analyzed. Results Fasting blood glucose, 2 h postprandial blood glucose, glycosylated hemoglobin, body mass index (BMI), waist circumference (WC), hip circumference (HC) and visceral adiposity index (VAI) in the observation group were higher than those in the control group (P < 0.05). The incidence rates of postpartum hemorrhage, premature rupture of membranes, premature delivery and fetal distress in the observation group were higher than those in the control group (P < 0.05). Fasting blood glucose, 2 h postprandial blood glucose, glycosylated hemoglobin, BMI, WC, HC and VAI of pregnant women were positively correlated with postpartum hemorrhage, premature rupture of membranes, premature delivery and fetal distress of neonatal and maternal pregnancy outcomes (P < 0.05). Conclusion The blood glucose level and obesity degree of GDM patients with obesity are positively correlated with neonatal and maternal adverse pregnancy outcomes.
